Personal Info:
Write down all your personal information; name, date
of birth, address, prescription info, your diagnosis, advance directive, and
any other important details
Self-care:
List what things you can do for yourself to care for
your emotional wellbeing, make your life more enjoyable & meaningful, &
whatever is important to you. Also include anything you know you need to do to
stay in recovery
My Warning signs:
List what warning signs you show when stressed,
suicidal, or heading towards a crisis;
Behaviors, thoughts, moods, feelings, or other
symptoms
Triggers:
What triggers you? List any situations, experiences,
memories, images, times of the year, or any other things that make you feel painful
emotions (anger, fear, frustration, sadness, anxiety, and despair)
Coping Strategies:
List what things you can do to comfort yourself, make
you feel safe, or calm you down, when you are feeling suicidal
Distractions:
What things can you do to temporarily take your mind
off your problems; these can be activities, places, people, etc.
Reasons to live:
List your reasons for living, future plans, goals, or
anything else that is important to you
Support Network:
Who can you contact when you are feeling suicidal? List
people who you trust, can talk to, enjoy being around, make you feel better,
and can give advice & support when needed. Also write down any professionals
or resources you can turn
Things I need & can ask for from others:
What are the things you need when you are in a crisis?
This can include help & support you might need from your family or friends
